MindMetrix QuickChart Tutorial
Supporting Your Psychological Testing Evaluation Documentation
Updated October, 2026
For the most up-to-date guidance on billing best practices and CPT code 96130 reimbursement, we recommend reviewing our latest guide.
Documenting a psychological testing evaluation takes more than recording test scores. Your note needs to explain how you interpreted the findings, integrated them with other clinical information, and reached your conclusions and recommendations.
MindMetrix findings may support this broader evaluation. QuickChart helps organize assessment information and provides a starting point for documenting the work you perform.
What Does 96130 Documentation Include?
- CPT code 96130 covers qualifying psychological testing evaluation services performed by a physician or other qualified healthcare professional. Documentation should reflect the clinician’s work, including:
- The clinical question and reason for the evaluation
- The assessment tools and relevant findings
- Interpretation of the findings and integration with patient history, interview findings, and other available clinical information
- Clinical conclusions and treatment recommendations
- Feedback provided to the patient or family, when performed
- Actual qualifying time spent on the evaluation, including start and stop times when required by the payer
At least 31 minutes of qualifying evaluation work is required for 96130, but time alone does not establish eligibility. Medical necessity, provider qualifications, documentation, and payer requirements also apply. Time counted toward another billed service cannot also count toward 96130.
How QuickChart Helps
QuickChart reduces repetitive data entry so you can focus on documenting your clinical reasoning. It provides:
- Pre-populated assessment information to help organize relevant findings and the tools used
- Time-entry fields to document the time you spent performing the evaluation
- Copy-to-clipboard functionality to transfer the template into your clinical note or EMR
The clinician must review the populated information for accuracy, interpret and integrate the findings, and document their own conclusions and recommendations.
QuickChart supports your documentation; it does not perform the evaluation or determine whether a service qualifies for reimbursement.
Quick Start Walkthrough
Open the patient's record: Log into your MindMetrix dashboard and view your patient list. Click the patient’s email address to open their record.
Find the QuickChart template: Scroll to Billing Documentation to locate QuickChart. Review the populated assessment information for accuracy and relevance to your evaluation.
Record your evaluation time: Enter your time in, time out, and total qualifying time, as prompted. Include only time attributable to the psychological testing evaluation and verify that the recorded total accurately reflects your work.
Copy the template into your EMR: Select Copy to Clipboard and paste the information into your clinical note or EMR.
Complete your clinical documention: Review and edit the template, adding your interpretation of the findings, the other clinical information you considered, and how that information informed your conclusions and treatment recommendations. Document feedback provided when applicable.
The completed note should explain your evaluation of the individual patient, beyond the assessment results alone.
Review and finalize: Confirm that the final record accurately reflects the services you performed and meets your practice’s and payer’s documentation requirements. If you are billing another service, ensure the documentation distinguishes the work and time supporting each.
How QuickChart Fits Into Your Evaluation Report
The MindMetrix report and an unedited QuickChart template are not, by themselves, the clinician’s psychological testing evaluation report.
The information you review, edit, and document in the chart may serve as your report when it contains the required elements and meets applicable payer requirements. You remain responsible for the interpretation, integration, clinical conclusions, and final documentation.
QuickChart is designed to make that process easier by organizing assessment information and reducing repetitive entry, giving you a useful starting point for documenting your clinical work.
